In order to provide you with prompt and complete care please be sure to bring the following with you:

  • Insurance information/cards
  • Photo identification
  • Any medical records pertaining to your visit
  • All current medications — prescription bottles preferred

Johnson and Murthy Family Practice asks that all new patients arrive at least 15 minutes prior to scheduled appointment to allow time for necessary registration paperwork before seeing the physician.

Insurance

Johnson and Murthy Family Practice work closely with area insurance providers to ensure access to quality care.

Please contact your insurance provider or Johnson and Murthy Family Practice with any questions regarding coverage.

Appointment Cancellation Policy

In an effort to have as many appointments available to our patients as possible, it is important to give our staff 24 hours’ notice if you need to cancel or reschedule an appointment.

Johnson and Murthy Family Practice may charge you a no-show fee of $20 if the 24-hour notice is not provided. Thank you for your cooperation in this matter.

After-Hours Coverage

In the event of an emergency after hours, please call 911 or proceed to the emergency room of your choice for evaluation. If you need to speak to a physician after working hours about matters that can not wait until the next day call the office number and the answering service will page the physician on call. Should you be taken to the emergency room for evaluation, the emergency room doctor should see to it that you receive appropriate care for your condition and will be able to contact our physicians.

Hospital Admissions

All physicians practicing with Johnson and Murthy Family Practice hold privileges at both St. Mary’s Hospital and Athens Regional Medical Center. Even though we have affiliate privileges, we do not admit patients.

Patients who need hospital admissions will be taken care of by “hospitalist” internal medicine physicians, who are available 24 hrs a day within the hospitals. We will resume outpatient care for those conditions upon discharge.
